I am using StereoTool, http://www.stereotool.com

I cannot recall how I came to the settings I am using now, I think I replicated from SAM and SPL when I did the transition years back.

Yes, Stereo Tool.  Very flexible.  In addition to the live plug-in in my encoder, I use some of it's features for clean up in day-to-day editing for distribution (as a VST plug-in in Audition, SAW, Studio One, etc.).
